Taylor Swift debuted her new “Patient Zero” music video during Sunday’s MTV Video Music Awards 2026.
In the short, Swift plays the ghost of Collin Farrell’s former wife trying to warn his new wife, Dakota Johnson.
The video opens with Swift and Johnson standing side by side at a graveyard wearing the same gray outfit. As the video goes on, it becomes clear that Swift’s character is invisible to others.
As Johnson and Farrell go about their day, Swift remains by Johnson’s side, singing the lyrics that warn of a toxic ex. Swift shadows Johnson and even soothes her as she throws up in the bathroom — sick with her husband’s poison.
In another scene, Farrell’s character is flirty with a woman played by Cara Delevingne. The scene then cuts to an intense fight between Farrell and Johnson while the ghost watches.
The next scene shows all three characters at a gala, where Johnson’s character finally sees Swift’s ghost and collapses in shock.
As they’re forced to leave the event, Farrell’s character snaps at his wife for making a scene. On the drive home, she apologizes to him and he accuses her of being drunk and humiliating him on purpose.
“I thought you were different,” he says, leaving Johnson’s character confused as she asks, “Different from who?”
The big reveal comes in a flashback of Swift and Farrell’s characters’ marriage — and it’s eerily similar to his current one.
Farrell’s character had poisoned and killed Swift’s character — and the grave Johnson visited earlier is revealed to be her predecessor’s.
Earlier this week, the pop star teased the new video during “CBS Mornings.” The 13-second clip unveiled that Johnson and Colin Farrell star in the short.
Johnson’s involvement in the music video comes after Page Six exclusively reported that the actress was using Swift’s NYC apartment to meet up with her latest rumored flame, MGK.
“That was just the best,” Johnson gushed of the music video at the “Verity” red carpet premiere in London on Thursday. “Working with your friends is like the most fun thing on the planet.”
“We’ve known each other for so long, we respect each other’s creative minds, and to be able to work alongside her and support and assist and be a person whom she tells what to do is wonderful.”
Swift, notably, directed the “Patient Zero” music video and worked with Oscar-winning cinematographer Emmanuel Lubezki on the project.
She purposely chose to debut the new music video during this year’s VMAs ceremony, which also recognized her with her brand new honor.
The 36-year-old is the recipient of the inaugural MTV VMA Artist Director Honors, which recognizes “groundbreaking music artists whose work behind the camera has expanded music videos and advanced visual storytelling.”
The honor cements Swift’s place as the most-awarded VMAs solo artist in show history.
Johnson, fittingly, presented Swift with the honor just before the “Patient Zero” music video debuted and the two watched it together from the crowd.
After the premiere, the musician took to Instagram and shared behind-the-scenes snaps from the making of the video with an emotional message thanking Farrell and Johnson for their work.
“I want to thank @dakotajohnson and Colin for trusting me with their time, taste, and talent (Giggling with your friend because you’re twinning for an entire shoot was a new and truly delightful experience). And Colin’s turn as a villain is even more impressive when you get to be around him. He’s an absolute light and as kind as they come. @caradelevingne, thank you for agreeing to play the lusty other woman. That’s what friends are for?”
Ahead of the show, fans noticed that Swift had been dropping hints about an upcoming project over the past few months, including in her “Opalite” music video and on social media.
Additionally, the Grammy winner dropped several Easter eggs during her surprise cameo at the Emmys 2026 in a pre-recorded skit with the cast of “Law & Order: Special Victims Unit.”
On Tuesday, Swift finally announced on social media that she was releasing her latest single, “Patient Zero.”
The next day, she sent fans into a frenzy when she announced “The Life of a Showgirl: The Encore,” a four-song expanded version of her 2025 chart-topping album.
The titles of the tracks are “Patient Zero,” “Cleveland!,” “Pink Clouding” and “Babylon,” and became available to stream everywhere on Friday.
